Your birthday is the perfect time to check in on your health.

Join the Sidney Health Center Birthday Month Club and take advantage of our Annual Health Screen at a significantly reduced rate. This affordable screening is designed to help you stay proactive about your health each year.

How It Works:

During your birthday month, bring your completed card to the Admissions Desk at Sidney Health Center. After completing registration and payment, you will be directed to the lab for your blood draw.

Helpful Tips for Your Visit:

• For the most efficient experience, plan to visit during the week of your birthday
• Fast for 10–12 hours prior to your blood draw for accurate results
• Drinking water is encouraged and will not affect your results

Important Information:

• Birthday lab testing is a self-pay service intended for screening purposes only
• Insurance and Medicare are not billed for these services
• Provider-ordered lab tests must be completed separately and cannot be combined with the birthday screening
• This offer is valid only for the registered individual
• Participants who do not use the service for two consecutive years will be removed from the program

Birthday Month Blood Screen: $50

(Valued at over $500)

The health screen includes a comprehensive chemistry panel to evaluate kidney and liver function, blood sugar, and electrolytes, along with a complete blood count to help identify anemia or infection. A lipid (cholesterol) panel is also included.

Health Screen Includes:

Glucose, BUN, creatinine, eGFR, sodium, potassium, calcium, chloride, CO₂, phosphorus, uric acid, total protein, albumin, alkaline phosphatase, AST (SGOT), ALT (SGPT), total bilirubin, cholesterol, triglycerides, HDL, direct LDL, CBC (no differential).

Optional Add-On Tests:

• Prostate Cancer Screen (PSA): $45
• Thyroid Stimulating Hormone (TSH): $40

Birthday Month Club members receive a reminder card in the mail with simple instructions for completing their screening. The discounted rate is available for three consecutive months, beginning with your birthday month.
